    Evaluation of patient perceptions of team based care in a Geriatric Oncology Clinic

    Purpose: To measure patient perceptions of collaborative practice in an interprofessional team providing geriatric oncology assessments to older patients with cancer. Background: The Senior Adult Oncology Clinic (SAOC) at Thomas Jefferson University’s Kimmel Cancer Center utilizes an interprofessional team approach to provide comprehensive geriatric oncology assessments and treatment plans for older patients with cancer. The importance of team-based healthcare delivery is well documented, however, experts agree that there is a need for more tools to assess the skills required to become a high-functioning team and a need to study the impact of collaborative practice on patient reported outcomes and satisfaction. For this study, we sought to evaluate patients’ experience and perception of our SAOC team function. Description of Intervention: Upon completion of a SAOC visit, patients were asked to participate in a short voluntary survey to assess team performance. The Jefferson Teamwork Observation Guide (JTOG) is a validated survey used with learners that has been adapted to elicit patient perspectives of five domains of interprofessional collaborative practice, including communication (C), values/ethics (V/E), teamwork (T), roles/responsibilities (R) and patient-centeredness (PC). The Patient JTOG includes eight competency–based Likert Scale questions as well as one open-ended question. The survey was administered on secure mobile tablets by trained research assistants (RAs) who were not part of the healthcare team. The study received exempt approval by our Institutional Review Board. Results: A total of 13 patients completed the survey. Seven respondents were female, and six were male. Seven identified as Caucasian, four as African American and two as other. One hundred percent responded “Strongly Agree” to a global question about the importance of teamwork in patient care (mean 4.0). Overall satisfaction with the SAOC team was 3.92 out of 4.0. For the eight questions relating to each of the five collaborative practice competencies noted above, the team received an average score ranging from 3.69 to 3.77 out of 4.0, for a global score of 29.66 (out of 32 possible), placing this team in the highest quartile of teams surveyed at our institution to date (n=407). In addition, all 13 respondents completed the open-ended qualitative comments with 12 out of the 13 being positive with multiple references to effective listening and communication, team coordination, and patient-centered care. Conclusion: The SAOC has a relatively unique model of providing interprofessional geriatric oncology assessments. The Patient JTOG tool was easy to incorporate into a busy clinic and provided valuable feedback to our providers, demonstrating that our patient’s perceive the team as highly functioning and effective.     Using Microservices to Design Patient-facing Research Software

    With a significant amount of software now being developed for use in patient-facing studies, there is a pressing need to consider how to design this software effectively in order to support the needs of both researchers and patients. We posit that a microservice architecture—which offers a large amount of flexibility for development and deployment, while at the same time ensuring certain quality attributes, such as scalability, are present—provides an effective mechanism for designing such software. To explore this proposition, in this work we show how the paradigm has been applied to the design of CONSULT, a decision support system that provides autonomous support to stroke patients and is characterised by its use of a data-backed AI reasoner. We discuss the impact that the use of this software architecture has had on the teams developing CONSULT and measure the performance of the system produced. We show that the use of microservices can deliver software that is able to facilitate both research and effective patient interactions.     Introduction: A number of large naturalistic trials have reported in recent years comparing second generation antipsychotic drugs with their predecessors. The conclusions they draw have rightly sparked much debate, but are these studies truly comparable? If not, which of them are most methodologically robust and are these the studies most suitable as a foundation for clinical care guidelines with a strong evidence base. We aimed to conduct a review of the current literature to establish the appropriateness of several recent major clinical studies being used as the basis for clinical guidelines.     LD4PE: A Competency-based Guide to Linked Data Principles and Practices

    The IMLS-funded Linked Data for Professional Education (LD4PE) project has developed a competency-based prototype referatory of Learning Resources for teaching and learning practices in the design, implementation, and management of Linked Data. This report summarizes the work of the project in developing: 1) an RDF-modeled “Competency Index for Linked Data” (Index) based on the Achievements Standards Network Description Language (ASN-DL) for describing formally promulgated competencies and benchmarks; 2) an openly available, web-based tool set to support the management of the Index; the generation of RDF metadata about Learning Resources; the packaging and arrangement of selected Learning Resources by users in “Saved Sets”; and the creation of learning trajectory maps expressing curricular structures or personal learning journeys superimposed over the competency framework through the integration of these elements as WordPress custom posts and taxonomies on the LD4PE website; 3) a set of cataloged Learning Resources that have been mapped to the competencies and benchmarks of the Index to support competency-based resource discovery by teachers, trainers and learners; 4) the LD4PE project website (http://explore.dublincore.net), which will be managed by the Dublin Core Metadata Initiative (DCMI) as part of its educational agenda; and 5) a set of Best Practices describing the infrastructure and policies developed for the project that others can reuse in mapping future knowledge domains in a similar manner
